Dental Hygiene Services for Residential Care Clients and Home Visits
Harbour City Dental Hygiene is pleased to offer a mobile dental hygiene service to clients who are homebound or in long term care facilities and cannot visit a traditional office setting to receive preventive dental hygiene services. Registered dental hygienist Heather Cooper has been practicing dental hygiene since 1996 and has acquired the special licensing from the College of Dental Hygienists of British Columbia to provide dental hygiene services to these often medically compromised clients.
Please complete all forms to the best of your ability and include a list of all current medications. This list can usually be obtained easily from the nursing staff. If the client has a dentist on record please be sure to fill that in on the chart, as any additional dental needs may be forwarded to that dentist.
Once our office has all forms returned, we will contact the POA/Guardian or Facility to schedule an appointment.
The first visit will consist of a comprehensive new client exam and dental hygiene treatment. Following this appointment, our office will contact you with information regarding the oral health of the client, any recommendations for oral hygiene aids (provided at cost) and the recommended time interval for maintenance dental hygiene appointments. If necessary and agreed upon, a referral for any additional dental needs will be sent to the client’s dentist on record. It is then the responsibility of the client or their POA/Guardian to follow up with scheduling appointments with the dental office.
